HOW TO ORDER SPECTACLES

STEP 1

  • Download our Script Order and Frame Example forms to send to the Optometrist and to help your patient choose a suitable frame …

STEP 2

  • Fill in the Yellow section of the form with all the details we need to process your order quickly and deliver the spectacles to you.
  • When you have an appointment for your patient, email or send the form to the Optometrist, who will then complete the Blue section of the form including the style of frame chosen.
  • Make sure the Optometrist signs the prescription section on the form.

STEP 3

  • Once you have received the signed Club Script Order Form, scan or photograph it.
  • Please ensure that the form is legible and not bigger than 6MB.
  • Complete the online form below.
  • Upload the scanned Club Sript Order Form and send it to our BrightSight office.
  • You can only upload one script form at a time. Repeat for multiple orders.

STEP 4

  • Check your email entered for a confirmation email from us.
  • When the spectacles are ready, we will courier them to your business address.
